I, the undersigned Guardian, affirm that the child below may ride Lonoke Baptist Church’s bus. I agree not to hold Lonoke Baptist Church nor any of its employees, agents or volunteers liable for any accidents or injuries which may occur due to my child’s participation in Sprout activities.
